If something about your care, your delivery, your bill, or the way you were treated falls short, we want to hear about it — and we would rather hear it early.
How to file a complaint with us
Ask for the person handling complaints, or simply say you want to file a grievance. You may complain verbally or in writing, and you may do so anonymously — though it is easier for us to resolve and respond if we can contact you.
What happens next
- We acknowledge it. We confirm we received your complaint and tell you who is handling it.
- We investigate. We review what happened, including records and staff involved.
- We respond. We tell you what we found and what we are doing about it.
- We act. Where we got something wrong, we correct it and change the process so it does not recur.

Our accrediting organization
ACHC (Accreditation Commission for Health Care) — accepts complaints about accredited organizations. Contact information is available at achc.org.
Florida Board of Pharmacy / Department of Health
Complaints about pharmacy practice or licensure may be filed with the Florida Department of Health, which administers the Board of Pharmacy. Consumer complaint information is available at floridahealth.gov.
Florida Agency for Health Care Administration (AHCA)
AHCA operates a consumer complaint process for licensed health care providers in Florida, at ahca.myflorida.com.
Medicare beneficiaries
If you have Medicare, you may contact 1-800-MEDICARE (1-800-633-4227), or your plan's grievance process.
