Who may use the portal
The provider portal is for licensed prescribers who have referred patients to Choice One Rx, and for practice staff those prescribers authorize. Accounts are approved individually by our team. Access is a privilege extended to your practice, not a right, and may be suspended or revoked at any time.
Your account
- Register with accurate information, including a valid NPI that belongs to you.
- Do not share your login credentials with anyone, including colleagues.
- Each person who needs access must have their own account.
- Notify us immediately if you believe an account has been compromised, or if a staff member with access leaves your practice.
- You are responsible for activity conducted under your credentials.
What you may access
You may access information only about patients your practice has referred to us and only for treatment, payment, or health care operations purposes. Accessing a record out of curiosity, or for any personal or non-clinical reason, is prohibited and may violate federal and state law.
Refill and renewal authorization
- Only the prescriber, or a person the prescriber has authorized to act on their behalf consistent with applicable law, may authorize a refill.
- Controlled substances cannot be authorized through the portal. Federal EPCS rules require a certified system. Send controlled substance prescriptions through your EPCS-capable e-prescribing workflow.
- You are responsible for the clinical appropriateness of any therapy you authorize.
- We may decline to act on an authorization where professional judgment or law requires.
- Each authorization is recorded with your identity, timestamp, IP address, and the exact attestation text you were shown.
Protecting patient information
- Access the portal only on devices you control and that are appropriately secured.
- Do not download, copy, or retain patient information beyond what your practice needs and its own policies allow.
- Do not send patient information to us by unencrypted email or text message.
- Log out when you are finished. Sessions also time out automatically after inactivity.
Accuracy of information
We work to keep portal information current, but statuses are updated as our workflow progresses and may lag real-world events. The portal supplements — and does not replace — direct communication with the pharmacy. For anything urgent, call us at 888.272.9055.
Availability
We do not guarantee uninterrupted availability. The portal may be unavailable for maintenance or for reasons outside our control. Always have a fallback path for time-sensitive matters.
Termination
We may suspend or terminate access at any time, including where we suspect misuse, credential sharing, or inappropriate access. You may close your account by contacting us.
