You only give us information through this site when you choose to — by filling in the contact form or by registering for the provider portal. Depending on the form, that may include your name, email address, phone number, professional details such as an NPI number, and whatever you write in a message field.
Information collected automatically
Like most websites, our hosting provider records basic technical information when you visit — IP address, browser type, pages requested, and timestamps. This is used for security, abuse prevention, and to keep the site working.

The referral form
Our online referral form is deliberately built so that the patient information you type stays in your own browser. It is not transmitted to us or stored on any server by that page. The form produces a printable packet you send by fax. The only network request that page makes is a lookup of a prescriber's NPI against the public CMS registry — no patient information is included in it.

Retention
We keep website enquiries only as long as needed to respond and to meet legal or accreditation requirements. Patient records are retained under the separate schedules required by Florida pharmacy law and federal regulation.

- You can ask us what enquiry information we hold about you and ask us to delete it.
- You can withdraw consent to be contacted at any time.

Children's privacy
This website is not directed at children and we do not knowingly collect personal information from children through it. We do serve pediatric patients — their information is handled under our Notice of Privacy Practices through their parent or guardian, not through this site.
