Choice One Rx is committed to treating every patient with dignity and respect. These rights apply to everyone we serve, regardless of who pays for your care.
You have the right to
- Be treated with courtesy, dignity, and respect, and to have your property and person treated with respect.
- Receive care without discrimination — see our Notice of Nondiscrimination.
- Be informed, in advance, about the care and services you will receive, and about any change to that care.
- Be told in advance what your services will cost, what your insurance is expected to cover, and what you may be responsible for paying.
- Participate in decisions about your care, and to refuse care or treatment — and to be told what may happen if you do.
- Have your property and personal information treated with confidentiality, as described in our Notice of Privacy Practices.
- Receive information in a way you can understand, including interpretation and auxiliary aids at no cost to you.
- Be free from mistreatment, neglect, verbal, mental, sexual, and physical abuse, including injuries of unknown source and misappropriation of property.
- Voice complaints or grievances without fear of retaliation, discrimination, or any effect on your care.
- Be informed of the identity and role of the people providing your care.
- Have your therapy delivered by qualified staff, and to be taught how to use it safely.
- Know how to reach a pharmacist after hours and in an emergency.
- Be informed in advance if your care will be transferred to another provider, or if services will be discontinued, and why.
- Formulate an advance directive, and to have your wishes respected as required by law.
- Examine and receive an explanation of your bill, regardless of the source of payment.
We ask that you
- Give us accurate and complete information about your health, medications, allergies, and insurance — and tell us when anything changes.
- Tell us about any other pharmacy you use and any other medication you take, including over-the-counter products and supplements.
- Follow the plan of care you agreed with your prescriber, and tell us if you cannot or choose not to.
- Store and use your medication as instructed, and dispose of supplies safely.
- Be available for scheduled deliveries, or let us know in advance if you cannot be.
- Treat our staff with respect.
- Tell us promptly about side effects, problems with your therapy, or changes in your condition.
- Meet your financial responsibilities, or talk to us if you cannot — see our financial policy.
- Tell us if you do not understand something. Ask again until you do.
If we cannot meet your needs
If we are unable to provide a therapy you need — because of a limited distribution restriction, a payer requirement, or clinical appropriateness — we will tell you clearly and help you find an alternative provider.
Raising a concern
If anything falls short of these standards, tell us. Our complaint and grievance process explains how, and how to escalate outside the pharmacy. Raising a concern will never affect your care.
